Epidural steroid injections are not effective for patients with lumbar spinal stenosis.

چکیده
Context Degenerative lumbar spinal stenosis is a common cause of low back and leg pain in older individuals. The evidence supporting non-surgical treatments, including oral medications, physical treatments and spinal injections, is limited. Despite the lack of strong evidence supporting the use of epidural steroid injections (ESIs) for lumbar spinal stenosis, their use has increased dramatically since 2000. This multicentre, randomised trial compared the effectiveness of ESIs with lidocaine to epidural injections of lidocaine alone.

Study Protocol- Lumbar Epidural Steroid Injections for Spinal Stenosis (LESS): a double-blind randomized controlled trial of epidural steroid injections for lumbar spinal stenosis among older adults
BACKGROUND Lumbar spinal stenosis is one of the most common causes of low back pain among older adults and can cause significant disability. Despite its prevalence, treatment of spinal stenosis symptoms remains controversial. Epidural steroid injections are used with increasing frequency as a less invasive, potentially safer, and more cost-effective treatment than surgery. However, there is a l...
متن کاملVertebral osteomyelitis: a potentially catastrophic outcome after lumbar epidural steroid injection.
BACKGROUND AND OBJECTIVE Epidural steroid injections are commonly used to palliate the symptoms of spinal stenosis. Deep tissue infection is a known potential complication of these injections. There have been no previous published cases of osteomyelitis without epidural abscess after such injections. We present a case in an elderly patient who presented only with persistent axial low back pain ...
متن کاملSteroid for epidural injection in spinal stenosis: a systematic review and meta-analysis
PURPOSE To investigate the effectiveness and safety of epidural steroid injections in patients with lumbar spinal stenosis (LSS). METHODS We performed a search on the CENTRAL, Pubmed, Embase and Cochrane databases up to September 2014. We recovered 17 original articles, of which only 10 were in full compliance with the randomized controlled trial (RCT) criteria. These articles were reviewed i...
متن کاملLumbar interlaminar epidural injections are superior to caudal epidural injections in managing lumbar central spinal stenosis.
BACKGROUND Epidural injections are performed to manage lumbar central spinal stenosis pain utilizing caudal, interlaminar, and transforaminal approaches. The literature on the efficacy of epidural injections in managing lumbar central spinal stenosis pain is sparse; lacking multiple, high quality randomized trials with long-term follow-up. METHODS Two randomized controlled trials of the cauda...

A 76-year-old woman with mild lumbar spinal stenosis (L4-L5, L5-S1) and lumbar dextroscoliosis, previously on 81mg of aspirin daily (discontinued at 14 days prior to procedure) and not on anticoagulation therapy, underwent a lumbar epidural steroid injection (T12-L1). Post-procedurally, she developed bilateral leg paralysis. A magnetic resonance imaging (MRI) study revealed a fluid collection c...
